Gout Pharmacology. Following hyperuricemia, the urate crystals get deposited in the joints and connective tissues and activate monocytes or macrophages via Toll-like receptor pathway mounting and innate immune response. This results in secretion of cytokines including interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) leading to endothelial activation and attraction of neutrophils to the site of inflammation. Neutrophils secrete inflammatory mediators that create an acidic environment, which further causes precipitation of urate crystals. Colchicine is used to prevent or treat attacks of gout (also called gouty arthritis). This condition is caused by too much uric acid in the blood. An attack of gout occurs when uric acid causes inflammation (pain, redness, swelling, and heat) in a joint. Colchicine does not cure gout, but it will help prevent gout attacks. Colchicine is not an ordinary pain reliever and will not relieve most kinds of pain. Colchicine is also used to treat a rare condition called familial Mediterranean fever (FMF). Colchicine may be used in 2 ways. Most people take small amounts of it regularly for a long time (months or even years) to prevent severe attacks or other problems caused by inflammation. Other people take large amounts of colchicine during a short period of time (several hours) only when the medicine is needed to relieve an attack that is occurring. The chance of serious side effects is much lower with the first (preventive) kind of treatment. Because some of colchicine's side effects can be very serious, you should discuss with your doctor about the benefits as well as the risks of using this medicine. This medicine is available only with your doctor's prescription. This product is available in the following dosage forms: Capsule Solution Tablet Gout is a metabolic disorder characterized by hyperuricemia. Asymptomatic hyperuricemia ought not to be treated until arthritis; renal calculi or tophi become evident. The cornerstone of therapy of acute attack is often nonsteroidal anti-inflammatory drugs (NSAIDs), barring specific situations wherein colchicine and corticosteroids do have a role. Usually NSAIDs with stronger anti-inflammatory action are used in high and quickly repeated doses and have a slower response response as compared to colchicine, they are better tolerated. Colchicine has a unique mechanism action. Intra-articular corticosteroids provide relief in acute attack and are given in patients having inability to tolerate NSAIDs and colchicine. Chronic gout requires treatments with drugs that either promote excretion (e.g., probenecid, lesinurad) or prevent its synthesis through inhibition of enzyme xanthine oxidase (allopurinol, febuxostat, etc.). Pegloticase and rasburicase, being a recombinant uricase enzyme, oxidize uric acid to highly soluble allantoin excreted in urine. In spite of these effective treatment modalities, question arises on their safety profile. Newer treatment options are being extensively studied especially interleukin-1 (IL-1) inhibitors but their approval is still pending. A drug to treat gout and other diseases, taken by mouth. The molecular pathology of acute inflammation associated with gouty arthritis involves several concurrent pathways triggered by a variety of interactions between monosodium urate crystals and the surface of cells. It works by decreasing inflammation. It binds to tubulin which is essential to microtubule polymerization in mitosis, and thereby disrupts inflammasome activation, microtubule-based inflammatory cell chemotaxis, generation of leukotrienes and cytokines, and phagocytosis. It inhibits activation and migration of neutrophils to sites of inflammation, leading to a net anti-inflammatory effect to ease gout inflammation. (Manifestation to treat) • gout: Less preferred to NSAIDs or steroids. • Behçet's disease • pericarditis • familial Mediterranean fever It has a narrow therapeutic index, so overdosing is a significant risk. Side effects can result from inhibition of mitosis. The poisoning by overdose begins with a gastrointestinal phase occurring 10-24 hours after ingestion, followed by multiple organ dysfunction occurring 24 hours to 7 days after ingestion. After that, the affected person either declines into multiple organ failure or recovers over several weeks. It interacts with the P-glycoprotein transporter and Cytochrome P450 3A4 (CYP3A4), which drugs such as erythromycin and clarithromycin inhibit, leading to death. It should not be given to people taking macrolide antibiotics, ketoconazole, or cyclosporine, or those who have liver or kidney disease, as colchicine blood levels is raised because its metabolism is interfered, potentially increasing its toxicity abruptly. (Side effects) Moderate doses: • gastrointestinal upset, diarrhea • neutropenia High doses: • anemia caused by damage of bone marrow • multiple organ dysfunction: kidney damage, low white blood cell counts, anemia, muscular weakness liver failure, hepatomegaly, bone marrow suppression, thrombocytopenia, ascending paralysis • alopecia (hair loss) • neuromuscular toxicity: seizures, confusion, delirium, hallucinations in children • rhabdomyolysis: A life-threatening condition caused by muscle breakdown and muscle death. • hypovolemic shock: Due to extreme vascular damage and fluid loss through the gastrointestinal tract, which can be fatal.

Colchicine is used to prevent or treat gout attacks. Gout is an inflammatory arthritis that is the result of the precipitation of serum urea into crystalized deposits of monosodium urea (MSU) in and around the joint. These crystals cause sever inflammatory arthritis, which is initiated by mechanisms involving various proinflammatory mediators, such as cytokines and chemokines. Colchicine affects the molecular pathology of gouty arthritis. And prevents the formation of microtubules. Colchicine interferes with the inflammatory molecules found in neutrophils and monocytes. Colchicine binds to both α- and β-tubulin to create a tubulin–colchicine complex that prevents the formation of microtubules. The state of microtubule polymerization can control numerous cellular functions, including intracellular organelle and vesicle transport secretion of cytokines and chemokines and migration, division, and regulation of gene expression. These actions influence the cell activity known to be involved in inflammatory pathways central to the pathogenesis of gout. However, with overdose, Colchicine becomes toxic as an extension of these mechanisms. Colchicine remains one of the traditional medicines used to treat acute flares of gout. The current preferred treatment involves a low dose of 2 tablets at the first twinge of joint pain followed by one additional pill 60 minutes later. At this dose many of the unwanted side effects of previously popular higher dose therapy with colchicine may be avoided. While gout remains the most common indication for colchicine, recent investigations shed light on its potential to prevent heart attacks, stroke and other cardiovascular disease. A study of colchicine presented at the American Heart Association meeting in 2013 demonstrated dramatic improvement in these conditions. Another larger study published in late 2019 revealed a 74% reduction in stroke and a 50% decrease in urgent hospitalizations for chest pain requiring revascularization in a population at elevated risk. Colchicine also seems to provide significant protection against recurrent atrial fibrillation following radiofrequency ablation. Another benefit of colchicine involves prevention of inflammation of the sac around the heart or pericarditis. Unfortunately colchicine may be associated with a wide range of adverse effects even including an occasional death. Careful consideration of kidney and liver function is mandatory along with evaluation of any drugs that may increase the risk of colchicine's side effects.

Although unapproved colchicine has been used for many years, FDA approved the first single-ingredient oral colchicine product, Colcrys, in July 2009 for the treatment of familial Mediterranean fever (FMF) and acute gout flares; in October 2009 Colcrys was approved for prophylaxis of gout flares (chronic gout). The approved prescribing information for Colcrys includes a new drug interaction warning, updated dosing recommendations and a medication guide. Pharmacists are reminded to dispense only FDA approved products. PHARMACIST Fi shares some fascinating info about a medicine for gout called colchicine (some colchicine brand names you might recognize include: Colcrys, Mitigare, or Gloperba in U.S.A.; Colgout, Lengout, or Colcine in Australia), in this first episode of her 2-part series of 'Some Need-to-Know Info!' about colchicine. Part 1 has a particular focus on things to talk about with your doctor before taking colchicine (many of which are also relevant to people who are already taking colchicine). Please note that the info in this series of colchicine videos and in this description section is for people taking colchicine for gout (not for people taking colchicine for other medical conditions). Please also note: PHARMACIST Fi recommends making medicine decisions together with a doctor - not on your own. Part 2 of this series (coming soon!) will look at potential side effects of colchicine - some of which are serious - so, although (thankfully!) most people on modern [lower] gout dose schedules do not get side effects - it is important to find out what side effects to look out for, because if they did happen, some of them would need emergency medical attention.
